Alumni Transcript and Records Requests
Bronx Envision Academy maintains records and transcripts for alumni of Bronx Envision Academy (2011 - Present) & Performance Conservatory High School (2004 - 2014).
We are able to provide official transcripts and other necessary documents for alumni.
Directions:
- Download and fill out the Student Records Request Form.
(On the form, it asks for the school district / borough / school number.
For BEA, this is 12 X 511.
For Performance Conservatory, this is 12 X 262.) - Email [email protected] and attach a copy of your photo ID (for example, a passport, state-issued photo ID, or an IDNYC ID) and the filled-out Student Record Request Form
